IMPORTANT CLAIM INFORMATION

• Prizes $600 or less do not require a claim form.

• Prizes $601–$5,000 require:

Completed claim form
Valid government-issued ID
Kentucky Lottery approval

• Prizes $5,001–$25,000 require scheduled appointments and can only be processed during Kentucky Lottery business hours.

• All payouts above $601 are generally issued by check.

• Tickets with tape, alterations, white-out, or damage may be denied.

• Stores reserve the right to refuse suspicious or improper transactions.

• Kentucky Lottery may deny payouts without explanation to stores.

WHAT TO BRING FOR CLAIMS OVER $600

• Original winning ticket signed by ticket owner

• Valid government-issued photo ID
(Driver License, State ID, Passport, etc.)

• Completed Kentucky Lottery Claim Form

• Social Security Number required by Kentucky Lottery

• Customer must be physically present

• No third-party claims or cashing for someone else

• Ticket must not contain tape, white-out, damage, or alterations

PAYOUT HOURS & APPOINTMENTS

• Claims $600 or less can usually be cashed immediately during normal store hours.

• Claims $601–$5,000 can typically be processed anytime stores are open, including weekends and after Kentucky Lottery office hours, once approved.

• Claims $5,001–$25,000 require scheduled appointments based on fund availability and Kentucky Lottery processing hours.

• Appointment claims must generally be completed before 3:30 PM Central Time on approved business days.

• Large payout appointments are subject to first-come, first-served availability.

• Cash payouts for larger claims are not guaranteed and depend on store availability.

TAXES & PAYMENT INFORMATION

• Prizes up to $5,000 generally do not have taxes withheld at time of payout.

• Kentucky Lottery will mail applicable tax documents to the mailing address provided by the customer.

• Stores do not provide tax advice and are not responsible for customer tax reporting obligations.

• Kentucky Lottery may withhold taxes on payouts above $5,000 as required by law.

• Stores do not determine withholding amounts and only follow Kentucky Lottery payout instructions.

• Most claims above $601 are paid by check.

• Payment methods may vary depending on claim amount and store availability.

STORE POLICIES & IMPORTANT NOTICES

• Stores reserve the right to refuse any claim transaction that appears suspicious, altered, unsafe, or inconsistent with Kentucky Lottery requirements.

• Customers attempting to cash tickets for another individual may be refused service.

• Kentucky Lottery may deny claims for reasons not disclosed to stores.

• Stores are not responsible for claim denials issued by Kentucky Lottery.

• Tickets containing tape, white-out, damage, excessive wear, or alterations may not qualify for payout.

• All claim approvals are subject to Kentucky Lottery verification systems.

• Customers must comply with all identification and verification requirements before payout processing.
